Nothing you enter leaves this page. There is no account, no tracking and no server — the questions, the scoring and the report all run inside your browser, and everything is gone when you close the tab. That matters here: the honest answer to several of these questions is not one you would want stored anywhere.
This is the short form of an instrument used on live recoveries. Answer as the project is, not as it is reported — the questions are written so that the uncomfortable answer is available, because a diagnostic that only offers flattering options tells you nothing.
If none of the four options is exactly right, take the nearest. The index is not sensitive enough for one question to change the outcome, and it is not meant to be.

What a proper look adds
This short form scores twenty answers. It is an estimate made from your own recollection — nobody has opened your programme. The full instrument asks forty-eight statements across the same eight weighted domains, and it is run against evidence rather than memory.
The programme you accepted and the one you were sent this month, compared activity by activity — so the gap this diagnostic can only estimate is measured.
Progress tested against defined quantities rather than judgement, including an unannounced sample drawn after the pack arrives.
A quantified position: a confidence level attached to the date being reported, not a single-point date.
Findings with numbered evidence and the decision each one requires — and a page recording what could not be evidenced at all.
That is the mobilisation review: defined scope, defined price, no ongoing commitment. Monthly assurance follows only if the review shows it is worth buying.
If the result above is in the distressed or critical bands, the honest advice is not to buy anything yet. Establish the actual position first — the second finding above tells you how — because every other decision depends on it.
